Thank you for your interest in becoming an egg donor at POMA Fertility! 

The simple application process consists of two phases:

1) The Prescreen Questionnaire (complete below): This is a short questionnaire that screens you on basic egg donor qualifications. Once you complete this portion of the application, you will receive an email with information regarding next steps.

2) Full Questionnaire: Once you submit the prescreen questionnaire and it is approved, you will be invited to complete the full-length questionnaire.

Before you begin, please review the basic requirements to ensure that you qualify for our program:

• Age 21-30
• BMI between 18 and 30
• Minimum of high school diploma or GED
• Does not use nicotine products and does not do recreational drugs
• Available for office visits between 7-9AM during the week
